Note
Objective
Join a note or a comment to a data in the form, in free text, with the possibility of associating a confidential status to the note.
Data
Note	The pulldown text zone is used to enter the note to be attached to the data.
Confidential	The check box is used to indicate if the note is confidential. Confidential notes will only be viewed by the authorized personnel who completes the form.
Actions
Ok	The Ok command returns to the previous page while saving the changes that were entered.
Cancel	The Cancel command returns to the previous page while ignoring the changes that were entered.
